Terms of Service

1.         Bellak will teach Student a program specially designed to help build a strong foundation in mathematics and algebra. The program is a supplement to regular school curriculum(s) and is a course comprised of hour-long sessions held weekly after normal school hours.

2.         The Student or his/her parents or guardians will pre-pay the tuition sum of $25 per remaining session, in full, in advance of the first session attended by Student and assist in enforcing all program requirements.  Payment in full is required. Full pre-payment gives a discount of $5 per session (for example: $100 for 20 sessions),  and bringing a friend who enrolls gives another discount. A payment plan is available, but without the pre-payment discount. Checks are payable to Bellak Math Prep, LLC.   All charges for returned checks are the responsibility of the payer. All payments are nonrefundable, except for Bellak-initiated reasons such as schedule changes.

3.         Cell phone use during sessions is prohibited, unless prior approval from Bellak is received, to meet emergency situations.  Students who are disruptive, inattentive or fail to adhere to required behaviors may be dismissed from a session, or upon repeated misconduct, from the program as a whole, without refund.

4.         The program is not responsible for Acts of God or environmental or other conditions which may require the cancellation of a session(s).  In the event of cancellation of a session, every effort will be made to offer make up session(s) at the first reasonable opportunity.

5.         In the event the Student is less than 18 years of age, the undersigned parent or legal guardian of Student agrees to indemnify, defend and hold harmless the program from any action, claim or damage to Student while participating in the program.
